Pre-Requisite and Instructions for Online Registration of Doctors (Step 3)

Pre-Requisite

Kindly arrange the scanned images of following documents :
  • Certificate and Marksheet of High School and Intermediate
  • Marksheets of B.U.M.S/ B.A.M.S.
  • Degree/ Provisional Degree of B.U.M.S/ B.A.M.S.
  • Provisional Internship Certificate issued by Board
  • Internship Certificate issued by Medical College
  • Internship Certificate issued by Hospital
  • Affidavit
  • Urdu Certificate(For B.U.M.S. only)
  • Voter ID card/Adhaar card/ Driving licence/Passport
  • Photograph (File Format .jpg, .png, .gif, Max. Size 20 kb, resolution 250px x 300px)
  • Signature (File Format .jpg, .png, .gif, Max. Size 10 kb, resolution 150px x 75px)

Instructions for Final Registration :

  1. Your Applicant ID will be used as your user name.
  2. Your Bank Reference No. will be used as your password.
  3. Take a final print of application form and submit it to Board of Ayurvedic and Unani Tibbi System of Medicine alongwith required documents, for final verification.
  • NOTE :
    1. Bank Draft for the verification of documents will be send through Registered Post at Board of Ayurvedic and Unani Tibbi System of Medicine, 7 Lal Bagh, Dhanvantri Bhavan, Lucknow-226001.
    2. Amount of Bank Draft will be made available through SMS after the final Registration.
