Pre-Requisite and Instructions for Online Duplicate Certificate Registration of Nurses and Midwives
Kindly arrange the scanned images of following documents
(Note : Scanned copy of photocopy is not acceptable)
Certificate and Marksheet of High School and Intermediate
Internship Certificate issued by Medical College
Voter ID card/Adhaar card/ Driving licence/Passport
Photograph (Max. Size 20 kb, resolution 250px x 300px)
Signature (Max. Size 10 kb, resolution 150px x 75px)
Process for Form Filling :
Step 1 :
Click on "Registration (Step 1)" link
Enter valid information in online registration form, then click on submit button.
You will get your Application Number and Applicant ID, after successful registration.
Step 2 :
Click on "Fee Payment (Step 2)" link after 24 hours of successful registration.
During transaction of State Bank payment enter your Application No. and Date of Birth then click on submit button.
Select appropriate payment option to pay fee. (Note :- If you want to pay fee through Cash/ DD/ Banker's Cheque, select option "Other Payment")
Print your E-Receipt after successful payment.
Step 3 :
Click on "Final Registration (Step 3)" link for final registration, after 24 hours of successful payment.
Submit hard copies of completed registration form alongwith documents to Board Ayurvedic and Unani Tibbi System of Medicine, for final verification.
