Pre-Requisite and Instructions for Online Registration of Doctors

NOTE :

Applicants are advised to check the details they are submitting in the Application Form and other Fee related details before Fee Submission. Fee once paid is neither refundable nor transferable.
आवेदकों को सलाह दी जाती है कि वे शुल्क का भुगतान करने से पूर्व आवेदन पत्र में दर्ज की गई जानकारियां तथा शुल्क से संबंधित अन्य विवरणों की पुष्टि कर लें। भुगतान के पश्चात शुल्क अप्रतिदेय व अहस्तांतरणीय है।

Pre-Requisite

Kindly arrange the scanned images of original documents for the followings (Note : Scanned copy of photocopy is not acceptable) :
  • Certificate and Marksheet of High School and Intermediate
  • Marksheets of B.A.M.S., B.U.M.S & B.N.Y.S.
  • Degree/ Provisional Degree of B.A.M.S., B.U.M.S & B.N.Y.S.
  • Provisional Internship Certificate issued by Board
  • Internship Certificate issued by Medical College
  • Internship Certificate issued by Hospital
  • Affidavit
  • Urdu Certificate(For B.U.M.S. only)
  • Voter ID card/Adhaar card/ Driving licence/Passport
  • Photograph (File Format .jpg, .png, .gif, Max. Size 20 kb, resolution 250px x 300px)
  • Signature (File Format .jpg, .png, .gif, Max. Size 10 kb, resolution 150px x 75px)

Process for Form Filling :

  • Step 1 :
    1. Click on "Registration (Step 1)" link
    2. Enter valid information in online registration form, then click on submit button.
    3. You will get your Application No. and Applicant ID, after successful registration.
  • Step 2 :
    1. Click on "Fee Payment (Step 2)" link after 24 hours of successful registration.
    2. During transaction of State Bank payment enter your Application Id(12 Digit Id) and Date of Birth then click on submit button.
    3. Select appropriate payment option to pay fee. (Note :- If you want to pay fee through Cash/ DD/ Banker's Cheque, select option "Other Payment")
    4. Print your E-Receipt after successful payment.
  • Step 3 :
    1. Click on "Final Registration (Step 3)" link for final registration, after 24 hours of successful payment.
    2. You will be required to login with your user name and password where user name will be your Applicant ID and password will be your Bank Reference No.
    3. Submit hard copies of completed registration form alongwith Bank Draft for the verification of documents(Only for universities outside U.P.) to Board of Ayurvedic and Unani Tibbi System of Medicine, for final verification.
  • NOTE :
    1. Bank Draft for the verification of documents will be send through Registered Post at Board of Ayurvedic and Unani Tibbi System of Medicine, 7 Lal Bagh, Dhanvantri Bhavan, Lucknow-226001.
    2. Amount of Bank Draft will be displayed after the final Registration.
